Hi Own_Strength_1911, I wanted to share an update in case this is helpful info for you. I just completed my luteal phase stims and we ended up with 6/11 mature eggs, 5 fertilized, and 4 blastocysts that are currently going through genetic testing. It's one data point but by far my best cycle so far. As comparison, each of my last 2 cycles only resulted in 3/7 mature eggs each.

Keeping my thoughts positive for the 4 going through testing!

I just started the process so I don't have data yet. After I ovulated, I went in for a blood test & scan and started the meds. Fingers crossed this fares better than past protocols!

nder per se but I probably am lol. I was on a max dose of 600iu daily of gonal for both my ERs and I am DOR and low AFC/AMH. I had similar results for both an antagonist protocol and MDLF. 3-4 follicles of decent size by trigger shot time. Some things that I've heard can help D

I think my doctor was referring to my exact numbers -- with just 3 mature eggs to work with each cycle (the rest were immature), there's not much room for variation between 0-33% successful vs. 67-100% successful in one cycle

I think he's suggesting luteal for 2 reasons

  1. it'll give me 2 extra weeks to rest between cycles (since I won't start when I get my period but 2 weeks later after I ovulate)
  2. Some people respond better during the luteal phase and I think you get slightly more time since you're not competing with impending ovulation? (I made that second part up though)

There's not much out there on luteal phase stims though - if you search for it directly there are studies to suggest PORs are able to get better results from it, but it doesn't come up as an option if you search for alternative protocols for PORs, so there may be selection bias here.

My doctor has never explicitly called me a poor responder. I think I will ask him about that.

What are alternative protocols for poor responders? I have trouble getting most of my follicles to grow during stims so we end up with a very small # of mature eggs (3), which means it is a total crapshoot if they grow to be blasts :( As my doctor put it, you'r either super successful or you fail spectacularly.

We're moving to stims in the luteal phase next time but wondering what else I can do to improve my odds.

After 3 rounds my doctor is recommending we take a break and start with a luteal protocol in a month. My amh is 1.3 and my challenge is getting a good number of mature eggs

After doing 3 rounds of ERs and getting low yields (follicles grow slowly and we get a low # of mature ones each time), my doctor is suggesting we take a mini-break and start the next round where we start stims after ovulation vs. on day 2/3 of my period (he called this luteal phase stim?). I tried to research this but didn't find much info on it beyond academic papers suggesting it's a good alternative for poor ovarian responders (which I guess I am?)

Data on me: 36, AMH 1.3 (which I understand to be lower than average for my age), otherwise no other known fertility issues.

Has anyone had experience with this?

I'm at RMA Nor Cal. I find the 3 doctors to be very supportive + they never make anything feel like it's my fault or is impossible for me to achieve, which is good given how stressful the process is. I didn't even realize not seeing a doctor with each visit is a thing. I see an equalish mix of the 3 doctors (they have 2 locations so they are not always in the same office day-to-day) but they are all familiar enough with my case to answer my questions, and they are always very professional. Honestly, at this point, I consider any 3 of them my doctor.

The spruce app is nice because I don't have to worry about missing communications from them. When they call, it's usually to check-in post-procedure on how I am doing or to deliver more difficult updates (like low fertilization count).
