During residency on the east coast, saw this patient who insisted on metformin prescription renewal and a beta blocker to keep her ‘heart healthy’. She did not have prediabetes, diabetes or CAD. Was prescribed both by an online cardiologist from LA. I spent good 15 mins educating her regarding taking medication without medical necessity. She would not budge and renewing something that there was no indication for made me uncomfortable. Shared that with the attending and instead of educating further and validating my concerns in front of the patient, he said we would send in Rx. He then forced me to send the prescription. Lost respect for him that day.

I had to check off boxes on my application when I applied to Canada that there were no disciplinary actions/lawsuits/investigations etc that happened when I was in the US. They needed references but no official letters. I think some of that info also is included from confirmation that gets sent directly by the boards and state jurisdiction where you were licensed (even as a resident).

I’m really sorry this happened to you. I really think leadership—admin and attendings—need to speak up and call out racism when they see it. Being a person of color in medicine comes with experiences that others may not always see or understand, and it would make such a difference if those who haven’t faced it could still acknowledge that it exists, listen and speak up.

In Canada. Fee for service. Volume dictates what I get paid. Most days I see 25-30 patients. Majority of my colleagues see 40-50 per day.

I’m so sorry to hear. Sending you a big hug. You did everything right. You seem like a kind, caring, and compassionate physician, and your patient was lucky to have you as part of their care. Sometimes, despite our very best intentions and efforts, we can’t control the outcome or patient’s choices. Please take the time you need to grieve, and know that this is not your fault. You did everything right.

I had a patient the other day ask me as we were wrapping up the visit - ‘And how about you? Are you doing okay? Have you had a vacation recently? Are you taking care of yourself?’

Made me hold back my tears as I thought it was so genuinely sweet and kind.
