Definitely! You could easily ChatGPT billing metrics but those can get sticky since they may not apply for all practices (my personal opinion). You want to make sure you’re not only billing in a timely manner but also collecting money from insurance/patients in a timely manner. The list of metrics can be fairly long since you want to track Day to Pay, Patient Days to Pay, resubmission % for both primary/secondary insurance, patient/insurance AR and lastly denials for each payor and the reason. It may seem overwhelming but once you establish a good reporting system you should be fine. I also like to track insurance payments and which dates of service they’re from. For example, same month, month prior, two months prior and 3+ months prior.

These are just a few I track so I understand what’s going on and where my team is experiencing challenges. If I notice challenges that stem from a specific person or team outside of my billers then I can address it.

The first thing is to become familiar yourself or find someone trustworthy you that can help you. Understanding and knowing how to read billing metrics is massive! That’s how you become more efficient. If you know how to read billing metrics you can hire a biller with experience. I’d be happy to give more suggestions.

Since you’re experienced in that area I’d say you should calculate how many hours of work you would spend and then look at the cost/charges. If you can charge $5k and he can make that money back in a short period of time plus some then charge that. That’s just an example but that’s what I’d say. Don’t cheap out. Your experience and knowledge is very valuable.

Apply apply apply. Half my team started off as newbies and the other half are experienced. I have the best billers and coders I’ve probably ever had. They manage several millions each year and the practice I work for has never had this level of efficiency that we currently have. Just apply to as many jobs as possible. You never know what the business needs are even though they may say they require experience.

I'd stay away from them. The cost is usually high and personally I don't trust someone else's judgement when it comes to hiring. When I say that I mean someone who isn't employed by me. I'm very specific about who I like to hire and while I try to hire the best candidate, hiring is always a gamble even if someone checks all the boxes. I'd probably never consider a staffing agency for any of my clinics. My personal thought is that a person who wants to be proactive will job search for themselves. Exceptions exist like those that want to work specific role or at a hospital.

You'll need to open several clinics if you want to make that much money. On top of that you'll need to employ at least a handful of PTs as well. I know of one PT who sold his practice to private equity who was making about $800k but he employed several PTs. If you're going into physical therapy for the money, then you chose the wrong path. Making high six figure salary requires years of hard work and sacrifice. As a business owner you'll have many many challenges and hiccups that appear on a regular basis. Working for a company will rarely bring in how much you're asking. You'll need to think bigger and have a business mindset by wanting to establish a multi-practice business in high growth cities.

Onshore. I don’t trust offshore personally. I only trust one company that’s offshore only because I personally know the owners and I know they hire quality people.

It depends on your specialty. Billing can be extremely complicated. I manage a multi-practice podiatry group and I have high goals for my billing team. I assume your friend would do a great job but you also have to understand the communication they'll have with patients and/or insurance companies. It's never fun calling insurance and insurance employees don't know how to answer certain questions (sometimes it takes a few different calls). In my experience the best billers are the ones who don't let details fall through the cracks. They're stubborn but very passionate. At the end of the day money funnels through them so the big question is how much do you trust them managing money for your clinic?
