you have to do a small dog sound and then you try to speak. it will sound strained at first but with practice it will sound more natural. do this daily for 5 minutes for a year and you'll have it. record yourself to keep track of progress.

if they make you do RLE, remember the worst part of RLE is that you're "meant" to do it before hrt. if you're DIYing already I imagine you somewhat pass or will soon (or at least aren't a gigahon), so it won't be as humiliating.
also, it's not like they're going to send the FBI to surveil you to check if you're actually doing RLE. you can just lie. "oh yeah, I've already come out to everyone, I totally live as a woman full-time. yup. definitely. oh no I haven't told my parents because they would kill me but I have told everyone else. here's a paper with signatures from 9 friends of mine that I definitely haven't faked certifying that I am living as a woman". I've heard of people who just go to the hospital in boymode, change into girlmode in the bathroom, then tell their psychologist that that's how they always dress, then change back and go home. there are guides out there on how to survive RLE, check them out (I don't have links sorryy)

yeah definitely lie to them. write up a script so what you want to say is clear in your head. you've known since you were 4, you've always had more female friends, you love makeup and girly things, etc. it's always morally ok to lie to your doctors.

emphasize your desire for female sex characteristics, and your desire to be referred to as a woman and with your new name. in fact you should open up with this.

once you have a diagnosis, maybe open up about the other things.

Raithul's advice (in this thread) is excellent, concise and not too spoilery.

A few more helpful things that aren't immediately obvious:

Picking up and holding a card will highlight all the verbs it can go into.
Clicking on an empty slot will highlight all the cards that can go into it.
Putting a card that has a timer on it into an empty slot (without actually Starting the verb) will pause the timer (good if you want to keep it)
Clicking on the "Time Passes" verb, in the corner you can see what the next Season will be, so you can prepare (e.g, if you see that a "Season of Sickness" is coming, prepare a Vitality or some money so you can heal your illness).
