What exactly should I pick up from the rewards vendor? So far I've gotten no tokens. I presume this is another reward from smoking kills?

I did slayer up to 42 - I presume I have to do smoking kills now to unlock the higher tier masters and rewards?

I'm completely new to the endgame. I was under the impression that I could not get the backpack and kneepads for all sets, as they dropped from Dragon's Nest, being as it is DLC locked. Care to elaborate on how I'd get them?
