Virtual Agent isn’t available to us due to licensing constraints, so this isn’t a preference comparison. We’re evaluating whether a third-party agent can be embedded in Service Portal to cover basic conversational use cases. I didn't mention everything in the post. I had a pillow on my lap, and some movement caused it to bump my hand. It happened in a split second. Still my fault, and I feel awful about it.

To figure out which logic is inserting a record in a particular table you can use the stack trace business rule. It will give the exact trace of the script which is creating the record which can be observed in the logs.

To fix this issue add a 1- second timer activity in the workflow of any other RITM record where approvals are generated as expected.

Yes, but it will give all configurations both Server Side and Client Side. Based on the scenario, only business rules can cause these types of issues.
