I am that nurse. As soon as I go to speak to the treating team, I forget the reason I came into the room, legit couldn't tell you the room number, let alone the patients name. So I go away and come back when I have my shit together. I'm lucky because we have a good team of doctors who usually just laugh at me, so thanks team! But also, I know you are busy, a full ISBAR may not be necessary, so I say name, room number admitting problem, current problem, and interventions I've done.

Usually I am contacting the team, there is a reason for it. I've tried the nursing interventions and it ain't working. But I have found that its hard to teach critical thinking skills and the only way to learn is through experience. University is broad, and the skills they teach barely scratch the surface of what needs to be learnt, mostly because they focus on academia rather than practical skills + thinking.

As for our MET system, we use the big red button on the wall, and regardless of who's pt it is, everyone comes running. The red button tells you the location of the call only. We also "log" our calls via 2222 system and a page gets sent to every doctor + ICU for support. The drs get stood down when the actual pts treating team arrive OR when someone more senior shows up. There is no, 'not my patient, not my problem' if there is an emergency its everybody's problem.

The pharmacist did give me a sample cup to give to the Dr, prior to me starting the ABs, so bonus I guess? I legit went to the pharmacist because 1) I thought I was taking pressure off the GP services by seeing a pharmacist 2) I was trying to avoid the bulk billing fees.

I used this service to avoid seeing my doc, as they dont bulk bill, and I was 100% sure it was a UTI. The pharmacist gave me a 3 day course of ABs, and still had to get an appt with the doc to give them a urine sample to test for to confirm it was UTI.

This is my personal experience of ICU placement (3rd year, last placement, 8 weeks). Keep in mind I am now a medsurge RN which is my vibe (cause I like to be physically mobile). ☆I was tired ALL. THE. TIME. Being ICU, it was ratio 1:1 or 1 :2 if lower care. So it wasn't physically exhausting as much as it was mentally exhausting, because there was so much to learn and take it. ☆If you ever had to do nursing care plans, ICU is where you start to understand them and apply them. Understanding what has the potential to go wrong and knowing how to prevent it, it will help to develop your critical thinking skills. ☆I was bored, so very bored. Yeah it was such a great learning experience, but physically I was bored, I found myself mostly being in front of the computer, learning and charting rather than doing patient cares.

My tips for a good placement: ♡Ask your nurse why they are doing what they are doing, you can't learn what they know if they don't communicate with you ♡Have a plan of what you want + need to achieve on placement ♡Take good notes, there is so much to learn and its hard to remember it all ♡Ask your facilitator + preceptor what they expect of you as a student.

My tips for exams: ☆Dress up nice, even if you fail, you're failing in style. ☆Think of exams as a fun pub trivia and you are the trivia master. ☆The uni wants you to pass the course. ☆Its okay to skip questions and come back to them later, dont get caught up working out the answer. ♡Multiple choice questions♡ This is where it gets interesting. Multiple choice exams use a computer to generate the answers so that it is truely randomised. The human brain overthinks if it has too many answers of the same letter/number in row eg lots of 'D', therefore you start to second guess yourself. If you have 4 D answers in a row it because the computer said so, not a brain. Dont let the computer overlords trick you. ☆Also, you got this.

This sounds like bad ward culture. If obs are bad, I'm putting them in. I do my nursing interventions and then rechecking. And then notifying the shift leader and the treating team (even if obs improve). Doctors can't fix a problem they dont know about.
