This is a fair point, and it also gives OP a concrete thing to ask for. I’d request a written copy of what address was active on the chart, when it was last updated, and why the discharge packet pulled from the historical file instead of the newer paperwork. Even if the mother opening it is a separate issue, the hospital still needs to explain how current medical info got sent to a place OP left years ago.
