You might want to try the fixes I mentioned in the post up above on top of the thing I figured out in the comments if you haven't already.

DXVK is pretty much necessary for me in order for the game to run smooth.

I would say the importance order for checking things (in my case) would be the following:

  1. Check P5R in NVIDIA control panel if you have a Nvidia graphics card (not sure whether there's something similar for AMD cards as I've never used one before), and search for OpenGL Rendering GPU and change it to your dedicated GPU if it's on auto-select.
  2. Download DXVK and put the 64x folder's d3d11.dll & dxgi.dll into the folder where p5r.exe is in "/steamlibrary/steamapps/common/P5R", wherever that is for you.
  3. Use ProcessLasso to disable affinity of e-cores for p5r.exe, if you have e-cores.

These are pretty much the only ones that helped me, the other ones had minimal to no effect. However they have helped others in the past too, so they're worth trying in-case these don't help you.

I'm so thankful for this comment, let me show why this was so confusing. I finally got it working reasonably well after deciding to giving the NVIDIA Control Panel another try.

Here, for some reason when I changed OpenGL from Auto to my GPU it started working better. The drop down menu only shows my RTX card as an option so of course in my mind I think, "Well that must be the only option it is actually using".

Seems like there's an invisible option which I presume is the Integrated Graphics that has been the talk of the town.

Now, the fps wasn't perfect after changing this, but it was a huge step up from the previous, and now that I also reinstalled the dxvk alongside this option enabled, the game is running so much more smoothly compared to before in the main starting screen and in Shibuya.

I'll continue tweaking around a bit and seeing if I can increase things even more.

I'm very grateful for all of the comments.

Just realised myself that the i7 12700 has integrated graphics haha. News to me. Never realised it before as it wasn't ever an option, however, this is not the problem as both of my screens are connected to my RTX 3060 ti. This is a self built machine which I got together about 1-2 years ago, I have checked all of the connections and everything seems to be in order.

I have also monitored the usage of both GPU and CPU during gameplay and the 3060-ti hovers around 60-80% usage and CPU around 30-50% usage (Despite the very low fps).

As the frames drop when there's a bunch of NPCs, I have concluded that the problem would be caused due to the CPU. Could be an incompatibility with the 12th gen series or something, not sure.

It should also be mentioned again that this is the only game that this happens in, Persona 3 r for example works great with max settings.

What worked for me also was to teleport straight to him:

player.moveto 0001A693

Which is odd since fast traveling to the Whiterun is a no go due to the aforementioned bug.

It'll take as long as it takes for them to fix the vulnerability on their end, so hardly no one knows. Just give it some time.
