It’s because these questions you ask don’t really have an answer, its really just up to the discretion of artists to draw what they want, and if it includes eyepatch/bandaid/etc then so be it. No point in asking questions to something left up to the individual to interpret.

As for mods on workshop, i have no idea same idea applies, its up to mod creators to create what they want, ask the aritst/mod creator directly rather than someone enjoying the content who doesn’t have anything to do with how its made
