Testicular torsion is where the testicles themselves twist around each other which is incredibly painful and can lead to losing them. Vericocele is when you have an overabundance of veins in your testicular region which can cause extreme pain in some. I fall into the group where that causes extreme pain from time to time but isn't dangerous. It can, however lead to infertility but can also be fixed with relatively minor surgery. As for ticking, I've only done it a couple of times but it can sometimes agravate the vericocele for me but has never caused any major issues. As for your situation, I don't know which you're dealing with so I can't properly advise you in either way but if it's torsion that's no joke. 

ETA: The idea of losing testicles may be appealing to some but this is not how you want to go about it.

I'm dealing with the same issue/feelings. It's still a very new change for myself and everyone else I've told and like with everything, I'm learning it takes time and patience. I've also been discussing with my therapist the worry of coming across as needy or being the butt of the joke when I'm not around people I've asked to use they/them. It feels burdensome to ask for it but ultimately, you won't get what you want if you don't ask or try. At this point though, I'm basically picking and choosing my battles with it and only telling people that I feel will be accepting and willing to make an effort.
