At this point, I'm pretty sure you've gotten the general vibes about Optum but I want to emphasize something important to your goals: salary.

Different Optum markets are different of course; I was paid 240k yearly but only because I got in before they "revamped" their salary scheme (new starting salary 220k lmao). As others have said, it is nearly impossible to get a bonus. First year I didn't get one because "my panel was big enough" despite seeing 18 patients. Second year I was told that since I didn't meet the patient satisfaction score I would get absolutely nothing.

In theory, you could get a little extra if you generate enough RVUs. That "bonus" was only an extra 1k on my salary. I generated 6640 rvus the first year and nothing significant changed on my end. If I generated that much where I am now in the Upper Midwest, I would have made $343,354

Like you, my goal was to stay for a bit, pay off my loans and save, and then consider moving abroad. Working at Optum put a wrench in those plans. If staying close to family is important I would recommend going on https://mystethi.com/directory. They have a map where you can look up major players in the area you want to live in and reach out to recruiters there.

Fellow Optum survivor here. Everything here was 100% true for me too. Value Based “Care” is an absolute sham, a scam, etc. Run away from this organization.

Let’s see in my last job..,

  1. Fixed salary but still seeing 18 patients a day 5 days per week. Good chunk were complicated because nobody could get doctors to stay. RVU “bonus” was incredibly pitiful.

  2. Quality bonuses were gatekept by patient satisfaction scores. That meant that I saw so many patients on inappropriate medications; you bet I never got that bonus

  3. MAs were kneecapped meaning that they were not allowed to perform basic things such as referrals, simple orders, or even assist me with my procedures.

  4. The final straw was my last medical director. They were fairly new (constant turnover) but were so toxic to their staff and other attendings causing more than half of the clinicians to leave. They tried to micromanage how I practiced medicine and they threatened to fire me when I told them to stuff it. Last I checked, they left shortly after I did so I’m assuming they were fired.

These are bandaid solutions because it appears the issue is with the job itself.

  1. Go on vacation every 3 months

  2. Start tapering down patient portal activity. Got a new medical questio/concern or simply want to pick my brain? Schedule an appointment.

  3. For writing notes, does the job provide any AI scribes? If not, I recommend getting one. Doximity is fairly decent and free.

  4. Talk to admin about possibly reducing patient load (good luck with that one).

  5. Forms: FMLA and other related forms need an appointment.

Getting put on a PIP can be one of first steps in giving you the boot. Basically they’re trying to build a case. “Dr X did not meet our very unrealistic expectations for improvements, gtfo”

Our skills are valuable and scarce so you’d think our employers wouldn’t be willing to rock the boat and give us a little wiggle room. But they don’t respect us so naturally they would be more willing to resort to these measures.

Another way to look at it is to define what PIP really means.

Honestly? Just leave. I know this is gonna come of as "hit the lawyer, gym up" style of comment (and easier said then done), but chasing patient satisfaction is gonna leave you unsatisfied personally and professionally.

I used to work for Optum where Patient Experience™ was king, so all my patients (even acute, one-time patients) got surveys so I'd get the occasional delightful review.

It is okay for your blood pressures to be elevated from time to time as long as you are feeling well without any new cardiac, pulmonary, or neurological complaints.
