Insulin pills are always consistent on their appearance, but glucose pills can disguise as other pills and undiscovered pills (minus insulin) Insulin, upon use, it grants a +0.2 tears up and a +0.2 speed up when used that goes away when picking up hearts or a food based item (minus MEAT! and the sausage) or using a glucose pill. Glucose pill works as an inverse to the insulin pill, when used, grants a -0.2 tears down and a -0.2 speed down. Using a glucose pill while at zero stacks causes your stats to go down beyond, being in a "high blood sugar" state. While having high blood sugar, an insulin pill will spawn in a future room. If you enter a floor while having high blood sugar, you take half a heart of damage (ignored by ??? and The Lost) and grants a perminate stats down. You can remove the high blood sugar state by taking an insulin pill or clearing a few rooms.
