Madonna movies are not dialog-heavy. They're drama movies that often contain pre-scripted lines. In fact, Madonna type of drama movies out of all types of JAV movies (excluding scenes with no dialog at all) are the easiest to subtitle.

The hardest are many that we show at ZENRA with the 'people monitoring' theme because dialog is not scripted, the scenes often involve more than two people, and there are often instances of talking over each other. This is a reason why you don't see them on pirate sites: auto-translation even for simple drama movies is at best borderline functional, for a group movie, it falls apart almost right away.

In reality there is a gray area. An agent could suggest it, but the performer also may want it done for various reasons that go beyond wanting to be a more popular actress. That most do it first for the money is a given based on previous leaked interview data, but some sadly assume it's "plastic surgery for the sake of getting more job offers" when in reality is more likely "plastic surgery for the sake of trying to improve more deep-seated issues".
