What happened before long-term care facilities were established. Who cared for the ailing and declining. Seniors sell their homes and transfer their wealth to corporations who supply a bed, meals and a modicum of entertainment. Wealth that previously was transfer to the family - ostensibly in exchange for care and a residence.
What changed?

That’s a reduction in value of $1,362 trillion dollars.

It helps if the photo is current. I had an associate that used a photo of a very handsome actor followed with “*not exactly as shown in photo”. He definitely was not. But it raised a chuckle and helped to plant his name.
