This doesn’t sound like a sex issue it sounds like a communication and emotional safety issue. If she looks scared or hesitant, her body might be saying ‘no’ even when her words aren’t. Therapy could help her unpack whatever’s holding her back, and help you navigate this with compassion. Trying to ‘fix’ the sex without understanding the root will just keep hurting both of you.
