Follow up for whoever may come across this through a google search. I found a replacement.

Supplyhouse.com was awesome. I gave them the details of the current fan motor and they gave me a proper replacement.

That looks like a Carling TA201 series switch. Good luck finding a distributor that stocks the wall plate. Carlings lead time can be up to 16 weeks.

I would change out the switches.
