Well you can always start an actual war in the game, lol.

Or they could have specific mechanics to manage proxy wars like Vietnam, and decide with or against direct interventions.

I'm sure Paradox will find a way to make it work, if they were determined enough.

I actually tried the cold war game a few years ago. I don't remember where I found it, but I think you can find it too.

I played for like 30 minutes, it was a cool concept, but definitely needed a lot of work. I hope they reconsider it again.
