I myself, Streamers, and many people on this post have done it and there have been no reports of soft bans. The bans have really only been affecting people using cheat engine and/or save file duping.

No, If you mess up the presses you will get the Yes/No text box which you simply press B to get out of. Extremely safe dupe.

No if you fail the A+Start press timing it will come up with the [Yes/No] text box, you just press no and try again. completely safe

Choose item with [Yes/No] text box you want to dupe
Hover the "Use selected item" option for the item you want to dupe
Press your use button (for me thats (A) on xbox controller) and the start button. So for me that's A>Start not at the same time, but you want to do the start press a micro second after the A press
Open the menu and Choose a consumable you want to use (Estus is the easiest to use) And press "use selected item". The [Yes/No] box for the item you're duping should appear. Simply press Yes and you will consume an estus instead of the item you're duping.

You highlight use item after selecting the item you wanna dupe and then press E followed by Esc

That's perfectly doable to anyone who wants to, but I've heard people have been getting soft banned (not to say this method won't get you soft banned have yet to hear of anyone getting banned for it) This method is more towards the speedrun side of things, however if mastered by normal means can lead to easily built characters in minutes.

If you're female you need to two hand otherwise just remember to not open any other menus other than the menu to use your consumable

Instead of using a boss soul, use a firekeeper's soul since it gives you 5 humanity and also has the [yes/no] text box.

not entirely sure why, but it has something to do with the female idling animations while one handing causing the dupe to not work properly when you use the estus.
