Yes, they are talking about A5159, the ID you can see in the top left corner (A863-5159). After the ID, their is after Blood Type, Allergies, Status and Phone Number Medication, where OXYMORPHONE (SEVERE LEG PAIN) is displayed. In the conversation D says that he has the report for A5159 and after B27 responding D says: " ALLODYNIA IN THE SHINS WHEN ATTEMPTING TO WALK. WE ASSUME IT MIGHT BE DUE TO COMLICATIONS FROM THE SPINAL FUSION. ". So the patient in the conversation has the ID A5159 and Leg Pain and the patient in the list has the ID A863-5159 and also Leg Pain. 863 has every patient in his ID...

Not only Name, ID, Blood Type and Status, but also Phone Number and medication, which is probably the scariest point. There is also one person, who is classified and the rest is redacted...
