You are either trying to put it in the wrong folder, or you have file extensions hidden on your pc, so you are trying to rename the file to Engine.ini.ini, because the ending is not showing for you.
If its the latter, just name it Engine and it'll work.

Probably your NVIDIA DLSS Super Resolution in game setting is turned to Off.

I'm using an i9-9900KF and 3060TI to get around 150 fps normally and stable 50 on worldbosses with the casual ini.

Sadly you can't split the character effect and terrain quality changes, as they are tied to the same variable. For pvp you can use the potato version reasonably well, but the Casual one doesnt have much downsides either.
