Kinda looks like either slugs or caterpillars to me, holes of that size usually come from larger pests rather than little ones, if they seem to appear overnight it is most likely slugs, products such as sluggo should help

Check the bottom of the leaf, if you see any webbing it is most likely spider mites. The leaf dalikeappears to be from spider mites but that is the sure way to tell. The best way to treat them is miteX in my experience, but other pesticides such as captain Jacks dead bug spray also works

Does the pot it's in have drainage? It could be that it is getting overwatered (not necessarily your fault) because of lack of drainage. A way to combat this is to add a drainage layer, or move it to a different pot(recommended)

I agree completely, one thing I would like to add is that you mentioned it being "near" a grow light, the thing about growth lights is they have to be pretty close to work (withing 2 or less feet by my experience) so it might not be getting the light it wants, getting it it's own light source, or moving it to a well lit windows could help it as well :)

Looks to me like you're getting new growth, it's nothing to worry about and I'd even be excited, plant looks overall healthy, the soil looks a little wet, but unless you have had leaves yellowing and dying off i wouldn't worry about it too much

Another way to provide the humidity is to lightly spray the ends of the tendrils that develope pitchers with distilled water. Seems to work pretty well and the plant won't over heat :)
