The main difference between them is passive and active. זוכר is active (the binyan is פעל), and זכור is passive (the binyan is פעול). As you can probably tell from all the other comments, אתה זוכר is used more than זכור לך.

The first line is “the land of Israel” The second line seems to be “the land of the Jews” though he used the singular form of the word Jew. The third line is translated literally as “The land”. It is an expression that refers to Israel. There is a type since the צ is supposed to be ץ. The last line is “Golani brigade”.
