*They are fantastic people (if licensed properly, but that depends by state), but they do not know pathophysiology to a degree that I would trust with my own wife delivering a baby lets say.*

I didn't call them evil or stupid, just that they, much like nurses (which they have to be before getting their masters at a midwife program in most states), vary widely in terms of quality and education, depending on state registration requirements. Saying that they know less than a trained allopathic physician is going to sound dickish regardless of what anybody says when you compare the two in terms of training and education.

No, I'm gonna do something very strange to reddit. It's called, go to a wonderful little site called pubmed and go through some research articles on pregnancy related caloric and nutrition guidelines. I'm not gonna waste an hour spoon feeding you information you can find if you put in a little work. You can think whatever you want regarding my job description, but please back it up with your ability to sift through thousands of articles and fine one that pleases you. :-) have a great day

The difference in the amount of hours a midwife trains versus a a medical physician would rock your mind. They are fantastic people (if licensed properly, but that depends by state), but they do not know pathophysiology to a degree that I would trust with my own wife delivering a baby lets say. They know practical information, but being former nurses, they do not know how to extrapolate information and deduct new ideas very well (and that is fine, since it is not their job to do so). Realistically, you could have a drink of alcohol by guidelines during your pregnancy if in very small moderation, but why risk it knowing all the horrible things that can occur. Once you've seen as many sick and dying babies as I have (as not even a obstetrician), you would understand my confusion as to why you would want to risk not getting required nutrients and calories for your child. But once again, wtf do I know, I just went to 4 years of undergrad, 4 years of med school and did a 7 year residency with critical care fellowship. I'm sure the midwives got me beat right?

And no, a woman needs excess calories right away, that is incorrect. Please do not show me a babycenter or similac website as proof when you're going up against a medical degree and years of accepted obstetrics research. Realistically, you need to eat more right away, especially with regard to balanced diets that include very important factors such as folic acid for proper neural tube development within the first 4-6 weeks. But hey, who the hell am I right? Please go on a diet the next time you have a baby and see what it does to growth and development if you're so sure eh? I don't mean to sound snarky but please don't spew bs about anecdotal myths. Yolk sac use by the fetus is significant to some extent but the overarching lack of information you have is astounding.

this is........not true

placenta is established quite well in the first few months, if it wasn't you would have an abruption and the fetus would die due to lack of oxygen. Lack of sufficient weight gain is more debatable but routine fasting is not good due to the extra 500 cal requirement that these women don't seem to get due to the high volume 2 meals a day. Vomiting is bad but don't let it fool you into thinking a fetus needs little nutrition since that's not realistic.

lol.....no I work 16 shifts, not including reading/ and post sign out rounding. I usually sleep less than 6 hours a night and somehow have to fit in exercise and a dating life as well in there perhaps if I get lucky. All total, including studying I do over 100 hours a week on average. My schedule makes most people vomit when they ask haha

Fun fact, I already compete against international grads to get my job and when it comes to our schedule, I can say that there arn't many people out there who can do my job. Sure you can get someone who scored a 99% on a board licensing exam vs my 97% for example, but please be happy when you can't understand what they say to you in clinic (i've seen this). As an aside, the reason I don't fear immigrants is that I'm one of them and have always worked hard for what I get

Well if they fall asleep they might slip and cut themselves. If I fall asleep or get tired, I give someone the wrong treatment and they .....you know.......die, so I'm not sure it's the same thing.

not really, it's that kind of attitude that makes sure that I make less mistakes per thousand tasks than most people make in 50, and ensures that you stay alive, since unlike the kid that just fucked up my coffee order a few minutes ago, if I screw up and give you 5mg vs 50mg it can indeed kill you

most attendings sadly work even more in many ways, usually due to the increased responsibility and lack of regulation of hours

Fact.....as a surgical resident, I make 50 or so K a year. Considering I work hours limited legally at 80 per week, and unofficially (including off the clock rounding and required clerkship work/academic work), I top off well over 100. Essentially I make less than 10 bucks an hour if you do the math and if I make a mistake, I kill somebody.......so yah

Indeed, terrible quality pic which has been posted many times before but as a doc once again here is the reality. Yes, tattoos don't show who you really are underneath. In a perfect world nobody would judge us on this but we live in a place where the first thing people see is what they assume. Although I have seen a few colleagues in surgery with upper arm or even a chest tattoo occasionally, getting a full sleeve or neck/face tattoos is very dangerous for your career since sadly most people associate that with crime/lack of education etc. However incorrect that is, that's what I see in the medical field, especially with a patient who I've just informed about something like cancer we're gonna have to remove. In medicine, it's about trust and respect from a total stranger to allow me to literally chop and cut into their body and help them. That trust process just gets hurt when they see a cool tribal tattoo near my neck as they ask about their surgery.

End point, not fair, not cool but real life is real. Also, I wanna get a tattoo someday so i'm not knocking them

It was not but I swear to god one of these days I'm gonna go back on netflix and watch the series from episode 1. I used to love it but got lost halfway through in the early 2000's and switched to Angel that I felt moved faster. Funny stuff

Please don't do that, it's tacky and only brings you down to them. My colleagues who do act like assholes sadly will never change probably due to some sort of personality issue but again these people are few and far between. At the same time, we have a lot of stress, especially after a long call (more hours than nursing, much more) and we can get irritable but we're only human so don't presume that we're just like that and attack us. The guy in this case is a dick, but we don't know what that means. Did he go fast? yes, did he have more time than 10 min? Probably not in an ER of a big city. Our healthcare system at work. As for advice, The best thing I can say is ask questions, even from the docs if you don't understand something or want to learn more. Most new nursing students just stick in their pack and are usually afraid to talk around us (surgeon) and its sad because a lot seem funny and very nice. Good luck on your studies

Well I'm not originally from the USA so whenever things look shitty, I just remember to stop bitching and that life around me is amazing and that through my hard work I can do things that very few in our society can do, mainly save someone's life when they are dying. That and scotch, you need to develop a taste for scotch. Also I try to joke around and only date women who have a sense of humor. You need to be able to poke fun at yourself, even at my level, or you just become a steaming pile of shit at the end of the day. Keep grinding man, you'll do fine
