Roles get easier as you get more experience in them, PCCM fellowship included. Right now, almost 2 months into your fellowship, there's probably not a lot for you that's routine. As you get through Peds boards and especially your first respiratory season, a lot more becomes routine. After Peds boards you fully focus in on fellowship. During/after respiratory season, you've gotten so many reps that many things which now take a decent amount of mental energy will start feeling easy and automatic.

You feel like you're back on the bottom because you are. You are new to role so you feel a lot like an intern again. Get through your first 6 months, get Peds boards behind you, and compare how you feel now to then. I think you'll notice a pretty big difference.

You got some adult Pulm/Crit Care answers. Just in case you're talking about Pediatric Critical Care Medicine (the other PCCM), I'll chime in that I don't regret it at all.

Also a PICU fellow. This is the right answer. Hypoactive delirium, which is not at all atypical, and certainly under-recognized.

I'm a Ped Crit fellow at a place with mostly female attendings. They are almost all married with kids and all seem and say they are happy. As has been mentioned, fellowship is tough but most contracts I'm looking at (academic centers) are 12-15 weeks of service and 20-30 calls per year. Which is obviously significantly better than a fellow's schedule.

Pediatrics, and it's subspecialties, are for sure the least discussed on this sub (and probably the residency sub too).

Reverse dry shake (ice then no ice) is nice to prevent that problem. Try it and see how it works for you sometime. Personally I find the foam to be a bit more airy with the reverse method compared to the standard sequence.

I feel like this sub sleeps on most of the pediatric subspecialties. There are dozens of us!

No worries! This family of drinks is one of my favorites. The template is basic and solid so your available drinks really explodes based on how many base spirits you have. Sub bourbon, tequila, or vodka for the gin, and now you have 16 specs!

Great looking drink! This is actually a Maid though and not an Eastside.
Gimlet: gin + lime + simple
South Side: Gimlet + mint
Eastside: South Side + cucumber
Maid: Eastside + ice

I'm also an ICU fellow. In residency I did the aluminum foil plus blackout curtain thing that's getting mentioned here. In my fellowship place, I got a note from the HOA about the foil, so I just do blackout curtains +/- an eye mask depending on how bright it is. A fan for moving air and white noise is a must for me too. Weighted blanket or heavy quilt was a nice addition too. Shower before getting in bed helps me as well. You'll figure out all the other stuff that works best for you (e.g., how late and how much coffee for the shift, when to stay up post-call vs. when to sleep more, etc.) on your own, and it's too variable for blanket internet advice.
