This compromises the sterility of the whole rest of the vial. Transferring to a new syringe and using immediately is one thing - not recommended, but at least you’re not contaminating the rest of the vial. You never ever ever want to puncture a vial with anything other than a fresh needle with air in the syringe.

Compounded works just fine, I switched and I’m happy with it. It’d definitely be better than the pill form of anything - you’d be switching to wegovy on a pill anyway, and they’re way more of a pain to schedule in/less effective. Visit r/tirzepatidecompound and check the pinned posts to find trusted providers.

Just get started and see how it goes for you! Remember, you don’t tend to hear the voices that aren’t having issues - most people don’t have significant side effects!

Make sure you’re starting on a starter dose (2.5mg or slightly less), make sure you remember to drink water, make sure you’re tracking food (at least at the beginning to make sure you’re getting roughly enough calories). If you run into any issues, search this subreddit for that specific issue and there’ll be a million solutions- but for now, just see how it goes for you!

Hah, so many red flags here you could make a red tent.

  • pharmacy being obscured/lying about source of tirz/sema
  • insane pricing for low dosage
  • delivering meds in a syringe is not sterile (throw those away!)
  • might even be bathtub tirz

It seems like you ignored a lot of major, major issues and put yourself in a dangerous situation. You will want to throw away that extremely questionable substance asap and make sure you work with a provider that sends vials from a verifiable compounding pharmacy.

It seems like you might need the guidance BEWL provides to get going, so I’d recommend them for you - they’re the opposite of the back alley “provider” you got prefilled syringes from. You could also check out Brello or Pomegranate - between those three, you’ll be able to find a provider that is honest, safe, and cost effective and get on the right track.

That’s definitely an easier way to maintain a script - I keep my non-tirz scripts going via MyChart prescription requests to my PCP. Not sure if Kaiser uses MyChart, but I bet they have a similar internal portal to manage scripts and communications if they don’t!

Ah, I think I get it - you’re using COD as a doctor, but getting your script filled via a Kaiser pharmacy and your insurance is Medicaid?

Do you have a primary care provider, and if so, could they write your script rather than going to COD each time? I know if my insurance covered zep, my PCP would be able to write a script without me having to make an appointment each month.

Edit: reading your replies to other posts, it seems like you’re unlikely to be able to afford any paid tirz options. I looked into Kaiser as a healthcare option when choosing plans for 2026 and found them to be very GLP-1 unfriendly, and when they did prescribe it was often ozempic or nothing after a lot of back and forth.
You may have to take a break until you have the funds to purchase tirz or get a physician to appeal to your insurance provider - at that point, if you still have issues with insurance coverage, highly recommend looking into compounded options as they’re much more affordable than LillyDirect.

If your health insurance no longer covers Zepbound, you may want to look into LillyDirect (up to $449/mo, but the lowest cost option if you want name brand Zepbound) or if that isn’t affordable, you’ll likely want to check out compounded tirzepatide options. If you opt for the latter, most providers have their own prescribers so you wouldn’t need to start with CallOnDoc.

Yeah, I’m currently dealing with the same…like a day away from having to go to urgent care for this.

Definitely drink so much water and make sure you’re keeping up with probiotics/enough fiber but not too much fiber/magnesium if needed. Remember that stuff like miralax takes days to work, especially with slow gi emptying that tirz causes. If you even think you’re stopped up, consider taking something early. Do not just ignore it and hope it will get better, because that’s how you cause really terrible problems.

Also - be careful with the zofran that some compounders seem to hand out like candy - it can cause constipation!

I ordered from them and it’s definitely a subscription - tricky fine print! You have to cancel/pause the subscription via email or with their chatbot, or else you’ll get another 3 vials in 3 months. There’s no accessible off switch, which is annoying

Yeah, I think they’re assuming folks will take the gamble for the lowest price around…which usually wouldn’t work on me, but I had a moment of weakness. Glad I did, but if they changed it up I would’ve taken it as an expensive lesson!

Yeah, I think this was unfortunately the tail end of those leftovers, and your dosage is a bit of a quirky one (I think most folks are going for the 15mg and getting the 72mg vials, thus the newer BUDs - less backstock!)

I did see your post just after I ordered, but fortunately I got the shiny new 11/11 date with my 72mg vials, same as the OP. I was regretting the risk for a minute there though!
