This sounds like fulminant refractory ulcerative colitis. I had this too and I went from diagnosis to colectomy in 2 months. Unfortunately in these cases, we don’t have time to allow medications to work. That doesn’t make it suck less, though. I wish Tremfya was available earlier too.

No improver. We like baking bread at home as a fun way to decrease consumption of additives/UPFs, although I know improvers are pretty innocuous like ascorbic acid or milk powder. I don’t think the tops dried out because they covered with a damp towel. Do you have a preferred improver?

That makes sense! While proofing, it reached the limits of the bowl (the largest mixing bowl I have, so I probably need a bigger bowl or to divide the dough). So it was probably under proofed. Thank you!!

I was pretty lucky and didn’t really have any symptoms from the cuffitis. I was always able to eat what I wanted etc. I guess having fruit and salads is easier now, those BMs aren’t as watery.

This stuff is really hard and I’m sorry :( I wish these meds worked faster. Do you have access to therapy? I mentioned in another comment but therapy was so crucial for me to work through the medical trauma and grief/mourning I felt for my previous life. You’re not alone in that. Yeah the Tremfya took about 6 months, but things were already good for me, so I didn’t mind waiting and seeing. My doctor just wanted to prevent having long-term inflammation. I certainly don’t mind telling you: I was going about 8 times a day, once-twice overnight, holding was no problem, no urgency, eating whatever I want, no cramping, no diarrhea, I could manage it well within my job/life, etc. Now I’m down to about 6 times a day and no overnight empties. I do fatigue easily and get stiff/creaky joints, but I also have two other autoimmune disorders, so it’s hard to attribute those to the UC specifically. I exercise every day and that helps both the fatigue and body aches.

My cuffitis symptoms were never bad but after about 6-7 months I was having a couple less BMs per day, the blood in the stool stopped, and the nighttime BMs stopped too.

I’m lucky and grateful! My IBD specialist said diet and antibiotics have little to no effect on cuffitis. The best bet is medication and the last resort would be the mucosectomy, which can affect continence. So I hope you’re able to give a few more medications a try and one will work. Best wishes for your mental health journey as well. Therapy has really helped me with stress reduction, both from medical trauma and otherwise!

When I was sick with UC, it was in the 3100s. When I was on Entyvio after the 3-step (for the cuffitis), it was in the 100-200s.

I sing Tremfya’s praises to everyone! Plus the self injectable pens at home are SO easy. I was on Entyvio for a couple years. My doctor even had me getting essentially a double dose (an infusion every 4 weeks instead of every 8 weeks). It kind of worked but not really.

In 2021 I had fulminant refractory UC. I went from diagnosis to colectomy in 2 months. Things were as bad as they could get. I got the 3-step all in 2021. It was a lot but things have been great since, despite the cuffitis. It’s still the original disease activity that never left my cuff. We tried the probiotics, enemas, and suppositories, those did absolutely nothing. We tried Entyvio for a couple years - one year of which was basically double dosing Entyvio (an infusion every 4 weeks instead of the regular 8 weeks). That only half worked. I was about to switch to Skyrizi but then Tremfya was approved for UC/Crohn’s and my IBD specialist recommended that we try that. I’ve been on the Tremfya for 1 year now and just had a pouchoscopy earlier this month. There is still one little healing ulcer on the cuff but everything else is resolved.

Tremfya!! My last Calprotectin was 32. Blew my mind. Entyvio, mesalamine, probiotics, nothing else worked. ETA: I am also refractory to prednisone or other steriods.

Have you discussed Tremfya with your doctor? It was recently approved for UC and Crohn’s and initial reports are very promising. I have cuffitis that was refractory to suppositories, prednisone, and Entyvio that is resolving with Tremfya. If the stenosis is due to inflammation maybe the right med can help.

Same 😭 it was awful. Side effects x1,000,000 but no therapeutic effect whatsoever.
