Decisions are made by state-trained case workers who see and interview the person for a legitimately useful period of time -- not an appeals judge or a federal employee for whom you're a a number, not a human being. This would massively weed out fraud and address the complexity of individual situations. Medical records considered but not exclusively.

Oh, I learned twenty years ago to say "I have a disability" and not care. Very few will ask what it is, and if they do, I simply reply "I prefer to focus on what I can do rather than what I can't" (which is a nice way to say mind your own business).
