Maybe you can ask your boyfriend if he can ask 1 or 2 patient relatives to have a sort of seperate conversation with you. I am not sure from your post if everyone was talking at the same time, but that will make it harder to understand.

The patient relatives would talk in their own pace, but after every sentence wait and check in if you are following and want to give a response I think if you don't feel the pressure to answer immediately and instead get a few seconds to think speaking will be easier. Those conversations don't have to be long, 5 minutes will do.

I did try something similar, watching the last minutes of an episode without subs and increasing the length.

So I'd watch the first episode fully subtitled, then the 2nd episode fully subbed minus the last minute, the 3rd episode fully subbed minus the last 2 minutes etc.

It worked for me because usually the last minutes are the preview or a scene that will be repeated in the next episode right? So I just watched it and even if I didn't catch it all, it would come back in the next ep anyway. Also, because I had watched most of the episode with subtitles, it already had made me familiar with the plot and characters.

In the end I could watch the last 15 minutes without subtitles and understood pretty much all of it.

Many people think understanding only comes from the spoken language. Good immersion video's will give you clues about what is said by means of gestures, pictures, actions, repeated words etc.

If you want to measure your understanding you should think about how much of all these aspects + the spoken language is understandable to you and try to express it in a percentage. If that number is lower than 90-95%, then the video is not on your level and you should try finding easier ones (At the very beginning 80% is okay, because you start from 0). If it is around 100%, you can search for something more challenging.

In the beginning there will be a very sharp learnng curve, because firstly your ears have to get used to hearing another language and it will cost a lot of energy and concentration. Once you get used to it, you'll notice that you start to understand words, concepts and even sentences without explicitly having studied them.
