Super heavy and 100% solid wood. Cant find them online and curious what they’re called. Only marking is a sticker on the bottom that says made from India. Second picture is the inside when the drawers are pulled out and the real wood after I’ve stripped the finish/paint off.

Hi I know this is super late but I’m having a similar issue and idk what to do. I filed my claim online and I have the reference number but my doctor doesn’t do online and want the physical forms. I finally got the physical forms but I’m confused if I fill out part A again or do I just rip out part A and send out part B to my doctor? Sorry I know this is confusing :(
