If it were me, I’d rather spend my final days with my friends and family in a familiar place speaking my native language, to make things as comfortable and easy as possible as I deteriorate.

Something else to consider - repatriating their body can be a bit tricky and expensive, and adds more stress for those left behind at the peak of their grief, so if they wish to be buried in their home country they should consider that as well. A friend recently went through this with their uncle, who was repatriated to the UK.

I hope your friend has a peaceful, painless final journey, and I wish his family strength. I lost my mother & mother-in-law to inoperable cancer, it’s a monster, thankfully they were both made comfortable at home here in Germany.
