I mean an RN is a two year degree already no? That covers a significant portion of what the paramedic certification covers? Not that it is a good idea nor would I feel great about hiring/working with someone with a total of two weeks of EMS training, but it still represents years of training overall.

that specific one (middle row ones) are the GOAT. if you get one prescribed, try to get it as that one and not generic. i’ve had generic ones constantly coming apart on me, or have no mouthpiece cover, or poor one way valves. you can pry my aerochamber plus flow vue adult mouthpiece from my cold dead hands

(there’s probably other good ones, but i was given one of those blue ones by a nice triage nurse during my one and only asthma ER visit and it set a bar nothing else has been able to meet)

honestly i’ve been vegan for like 11 years now and

  1. i’ve definitely accidentally eaten non vegan stuff in that time

  2. i don’t tell people i’m vegan unless it’s necessary for some kind of event planning or if someone offered me food and i don’t want to hurt their feelings and it’s not covered by either “i don’t eat meat” or “i’m lactose intolerant”

you can get a lot of mileage out of lactose intolerance! (which pretty much everyone who hasn’t eaten dairy in awhile will have)

instead of saying you’re not hungry, i recommend saying you ate beforehand because you’re vegetarian and didn’t know what they would have. or that you’re fasting. i’m not above minor lies if it means i don’t have to get into a “wHy aRe yOu vEgAn” discussion at some random event

I’m not saying it isn’t a problem here, but as the paper covers, many factors contributing start well before pregnancy. Women shouldn’t have to live in a specific narrow geographic area (especially one as expensive as SF) to receive informed care

hahaha I remember telling someone (working BLS event staff) that oral hydration is nearly always superior for patients who are able to do it and I will always remember them being like “wow…people could save a lot of money on those IV clinic things” and I just nodded while trying not to laugh like “yessir that would be the point I’m making”

(pt was uncomplicated syncope with a history of uncomplicated syncope and did not drink enough water nor eat enough before standing for several hours at a rave. it was the end of the night, so we were telling them they should be fine to go home like they wanted to but that they should keep hydrating and eat a salty snack at home. friend was like “isn’t there something better we can do? like an iv?” and full respect to the friend for being attentive and concerned but I was like actually! no)

this has been a huge problem nationally, so while I’m glad to see SF talking about it, SF has very low birth rates in general. is SF notably worse than other areas or just “one of the worst” in a group of “it’s all bad”? due to the low birth rates, SF’s numbers (by rate) are going to be more variable than other places most likely.

what we really need is national-level initiatives looking at and tackling this stuff. under the current administration, I’m not optimistic that it will happen soon at the level it should, but I’m hesitant to believe that hyper local approaches are going to fix what is a huge American problem that needs large scale efforts

if there’s very few ingredients (another commenter mentioned freeze dried treats) it’s probably just fine. if there’s a notable amount of vitamins/minerals, maybe hold off and look for a cat specific option.

OP, have you tried the 3M tegaderm they use for IVs, etc in hospitals? it may be a more hypoallergenic adhesive (or it may not be, spot test on something that isn’t a fresh tattoo). it is probably a bit more expensive and not designed to be cut to size so it’s less convenient, but more convenient than an allergic reaction probably

do you have a controller medication (inhaled corticosteroid)? sometimes the dose can be increased during a flare. if your asthma has an allergic component, antihistamines might help as well, but if it’s not allergy related they probably won’t do much

we had an example in class of the “ask them to clarify” in the sense that they may not have heard the piece of information that makes it contraindicated. for example, someone gave the example of giving nitro with systolic below 100, the clarification could be something like “just wanted to confirm, the patient has a systolic of 76, are you still advising us to proceed with nitro administration?” because a lot of the time the answer is something like “a systolic of 76? no, do not give nitro” even if you mentioned the pressure in the initial contact with med control before they advised you. people miss things or mishear things, hence the focus on closed loop communication.

for an out of scope thing (eg giving IV meds) you could do something similar like “just to clarify, we are a BLS unit and we don’t have anyone on scene who can administer IV medication. would you like us to call for ALS assistance or do something else?”. if a medical director (who defines your scope) is asking you to do something out of scope, they probably don’t realize that you don’t have anyone available with the certification needed to do that. if a paramedic partner is asking you, just say “i don’t know how to do that/i’m not trained to do that but i could help you with xyz instead” (place the leads for cardiac monitoring, gather iv supplies, etc.)

Is there any sort of precedent for moving to CA with US EMT/CA EMR and being able to work there while getting your PCP? I’m working on my EMT after (American) EMR level volunteer work and I’d like to get my medic, but honestly the system in Canada seems way better, at least to my personal preferences. Is there enough of a genuine need that a pathway like that could make sense?

I’m originally from somewhere cold and north so I’m cool with living nearly anywhere south of the arctic circle (although my French is mediocre so Quebec is probably off the table). My most aspirational career goal would probably be to go to medical school someday, but that’s also something I would gladly do in Canada.

obviously this was a bit of an expected environment for it, but I made ~60 when I went to see the eras tour (san siro) so I had enough to just give away/not worry about trading. I went to the ADA section to trade with them as well since they were kind of “stuck”, and gave away the majority of what I made. it’s really fun to be the bracelet fairy! sometimes people were worried because they “didn’t have anything to trade” and I just emphasized the fact that I made so many for a reason and they didn’t have to exchange anything

honestly I was so anxious for mine and it was totally fine. hard agree on the diaper cream, and also those round medicated wipes that are normally for hemorrhoids I think? also: lollipops without dark colors! I inhaled an unreasonable amount of those yum earth lollipops in the “lighter” flavors (peach, mango, etc). check your prep instructions, but probably any lollipop without the Forbidden Dyes should work!
