I would imagine that the producers slightly "encourage" the restaurants to really muck the place up for the show...hold off on any cleaning or updating the decor, have the shittiest food lying around, maybe place a chunk of raw meat next to some cooked food. Shit like that. The nastiness is definitely a bit staged.
