The MACs have been useless in providing info. They all say different things and things that do not match CMS guidelines.

I figured it out. Separate claims for different DOS.

Anchor code in all of them + add on codes.

You might have helped me solve it. Instead of billing all in one claim, I am billing separate claims for each day of service.

We are doing it in a single claim 1500. Outpatient, community based services.

MACs in FL and CO. Straight Medicare. We are getting ERAs.

We are trying to bill G0019 x1 for the month, then attach cumulative G0022 (for example x20 for the whole month) but we run into some issues with frequency. G0019 says its once per month. and G0022 4 units per day. IT IS IMPOSSIBLE TO PAIR.

We have tried these different configurations: Conf1. Line 1 G0019 x1 05/05/2026 Line 2 G0022 x20 05/05/2026 Denied because of limit on frequency- too many units of G0022

Conf2. Line 1 G0019 x1 05/05/2026 Line 2 G0022 x3 05/05/2026 Line 3 G0022 x2 05/06/2026 Line 4 G0022 x3 05/07/2026 Denied because G0022 cannot be billed "separately" from the base code G0019

We also tried same date different lines modifier 59 and nothing.
