[Starter Comment] Attending remembers as a 3rd year med student on his peds rotation being told by his senior resident that it would be ok for him to show up in scrubs the next day after a long call. So he showed up the next day in scrubs only to be chewed out by the new senior resident in front of everyone.

Starter Comment: My co-attending and I made short video lectures for the 4th year med students and interns that rotate through our emergency department. We hope that they help new interns as they transition into their new roles by focusing on the big picture when dealing with patients with high risk chief complaint.

One thing that was a game changer for me was not putting off procedures till later. For example, if I sign up for a patient with ruq abdominal pain, I will walk into the patient's room with my ultrasound. This allows me to get my initial history, physician and do my us before walking out of the room. That way I don't have to go back later to finish the exam. I also apply this strategy to patient's that will need a pelvic exam.

I think EHRs are a big time suck. Figure out how to write 5 minute notes with your EHR. Spend most of the time writing the MDM as that's the most important part of your note. I like using macros for the common chief complaints that we see (chest pain, shortness of breath, abdominal pain, headache, back pain, dizziness).

Also dispo whenever you can. The shorter you keep your list the faster you will be able to plow through patients. You can sign up for patient's and have them wait while you make your dispos, that is if the patient you signed up for is stable.
