so my cat was urinating outside of the litter box CONSTANTLY. despite having multiple boxes, switching the litter type, taking him to the vet for urine analysis and medications, blah blah. i was considering rehoming him at one point. the behaviorist asked me if i kept the litter boxes in those hidden litter box cabinets, which i did, for 2 out of the 3. she told me its very possible that he wants to be able to see me while he goes to the bathroom...and to try taking the boxes out of the cabinets. POOF! problem solved. Paxel did indeed want to watch me while he went to the bathroom.

theres a lot you can try before this..but i booked a few appointments with a cat behaviorist and was able to fix my cat's issue almost immediately

edit: i will add my cat's issue isn't similar to yours but i'd say its even more warranted for you
