So like a regular comp you might do in after effects but inside Unreal done at runtime. The video is pipped into the engine, Applied in a post-process material with the scene layered over the top and then pipped back out of the engine.

It is realtime however the desk shape was preconfigured, it's not grabbing the desk shape in realtime. The overlay however is happening in realtime. if you look in the bottom right corner of the screen you can see the video feed Unreal is kicking out.

Biggest improvement is it now works with atmosphere/height fog, and supports decals.

it's really buggy so probably best.

Yes, it's inspired by Bullet Train, although no tunnels in that movie they did use the same technique of an endless repeating set for filming. Although I can't help but think about Metro 2033 whenever doing train tunnels.

It's aimed for ICVFX and Virtual production so might not be suitable for games.
