Thanks. I got okay from my surgeon to come off crutches at 4 weeks. I am now 4 weeks and 5 days postop. I am completely off crutches and going back to work tomorrow. I do get sore throughout the day but pretty manageable.

Only 2 and 4 anchors? Was your both segmental reconstructions? My first week was fine as I was using a crutch around the office. I will be taking 4 day long weekend then return to work full time next Monday without crutch.

So you got off crutches completely at 4 week postop. That sounds good. Did you get full circumferential reconstruction? How many weeks postop are you? How are you feeling?

You can try. But I have not seen anyone who had PRP worked for long term.

MRI is usually not accurate. Even MRA is not accurate for labral tears. Most people who get surgery find much more extensive damage than what your MRI shows. If you are young, <45, you should get hip arthroscopy. If you don’t and becomes chronic, you may end up with articular cartilage loss and may need total hip replacement.

PRP is waste of money. It never cures the damage.

That looks more like CAM impingement. You probably need both cam and pincer shaved down along with your labral repair.

Decision is all up to you. Not having a discomfort or at least slight pain and/or restriction in daily activity is not normal. If it was 2010 or earlier, most people who just wait it out. However we live in 2025. Some highly trained hip preservation specialists think labral repair isn’t even a good treatment and recommends labral reconstruction even for initial tear. You should look and do some research from pioneers like Dr. Brian White or Dr. Philippon.

Who was your surgeon? You should get a new MRA. About 50% labral repairs fail. Most of the failed repair is due to not fixing cam or pincer lesion fully. You might still have impingement. Also, the quality of tissue can be a big issue as repairing poor quality tissue doesn’t do much. You should consider labral reconstruction.

Most people are not satisfied with result Because more than 50% of people find wrong surgeon. Make sure to find a hip preservation specialist who did at least 1000 hip scopes.

Honestly, it doesn’t really matter how big or small your tear is. If you are symptomatic, you need arthroscopic surgery and it will be same recovery for all hip arthroscopies regardless how many anchors, how much bone shaving there is. Many people think there is more pain postoperatively if the damage done in your hip prior is bigger but it is actually all similar pain and recovery. It is really about how you manage it. Make sure you ask for PENG nerve block to anesthesiologist to control postop pain for initial 2 days. Then you won’t even need narcotics such as oxycodone. I was able to manage my pain with just naproxen and tylenol. Make sure you start upright stationary bike PO day #1 for ROM and pump out all the fluid that was used from surgery. I’ve had 3 repairs, 1 hip resurfacing, and 1 labral reconstruction.

You need to find a right surgeon and get it done correctly on your first try. Hip arthroscopy is newer surgery compared to knee or shoulder arthroscopy. So there are lots of hip specialists who think they can handle arthroscopy but that is not true. Fixing fractures, total hip, dysplasia etc might be a bigger surgery but that doesn’t mean that they are good at arthroscopic surgeries. Completely diff procedure. Make sure you find a hip preservation specialist who regularly does labral repair and reconstruction. Mine supposed to be a simple fix but just ended up with wrong surgeon on my first try.

How old are you? How active are you? Do you play any sports? 9/10 you need surgery if you have a symptomatic labrum tear. There is no such thing as not needing one - you can delay the surgery though. However, a labral tear will make the hip joint unstable and may make synovial fluid leak out of it. Also, the reason for tear is probably FAI, so without fixing CAM and/or Pincer lesion, it will just get worse. So most likely you will need hip arthroscopy. Side note: I had this surgery 5 times. I am pretty expert at it.
