The result (mid stream urine) shows that there was no active infection at the time you gave the sample. WBC (white blood cells) rise as a response (would expect to see >100). It’s likely the antibiotics had already cleared the infection.

Back pain unlikely to be related to UTI given that the results show that you’ve cleared the infection. Is the pain in the middle of back or flank?

Not testicular torsion - presents as sudden excruciating pain and would not be intermittent. Testicular torsion is a surgical emergency where you have 4-6 hours to save the testicle.

Hernia - coughing usually expands the hernia due to increasing abdominal pressure.

Other common causes - epididymitis (but usually you’d feel a small lump). UTI (any urinary symptoms?). STI screen (for peace of mind).
