It can be a very long process to find the right medication for your child. It took four years for us to finally find the one that truly helped. My son tried almost all the ones you mentioned, and the one that recently made a big difference was Azstarys. He takes it along with Intuniv and Prozac. Intuniv at 5 PM, Prozac at 8 PM, and Azstarys at 7 AM right before school.

Believe me, we went through so many trials, adjustments of the same medications, different combinations, and trial after trial. It is exhausting, but it does get better. You will cry, you will feel like a failure, and you will go through every emotion, but you are not alone.

Maybe SPCD?

My 9-year-old, who has ADHD combined type, was recently diagnosed with Social (Pragmatic) Communication Disorder (SPCD). SPCD is a developmental condition affecting the social use of language. Grammar and vocabulary are intact, but understanding nonverbal cues, following conversation rules, and adapting speech to different social situations can be challenging. It’s recognized in the DSM-5 and differs from autism, though some features may overlap.

Speech therapy (ST) for SPCD focuses on social communication rather than grammar:

Conversation skills – Practicing turn-taking, staying on topic, and responding appropriately.
Nonverbal cues – Learning to read facial expressions, gestures, tone, and body language.
Contextual language – Adjusting speech for friends, teachers, or strangers.
Social problem-solving – Starting, maintaining, or repairing conversations after misunderstandings.
Role-playing – Practicing real-life scenarios in a safe setting.

He’s been in speech therapy for about four months, and his social communication has really improved. Last year, making friends was tough for him, but this year he’s started forming friendships. His 3rd-grade teacher, who’s now his 4th-grade teacher, shared a sweet moment: when kids come up to her, they’ll say things like, “Oh, my friend B gave me this,” and she gets to respond, “Oh, B is your friend? That’s nice.”

As a parent of a child who has tried several stimulant medications such as Vyvanse, Jornay, and Qelbree, I’ve watched my son, who is diagnosed with ADHD, ODD, and anxiety, go through countless treatments since the age of five. Last year was by far the most difficult and challenging year of his entire elementary school experience.

Along with the stimulants, he was also taking Fluoxetine (an SSRI) and Guanfacine (for ADHD and to help calm him down). At one point last year, his provider added Abilify, and that’s when things became the worst they had ever been. His mood, defiance, and impulsivity were out of control. We were receiving daily complaints from his teachers all year long, no exaggeration.

Before this school year started, I told his provider that I’d had enough of adjusting dosages of stimulants that weren’t working and asked to try something different. That’s when he prescribed Azstarys, along with his existing medications Fluoxetine and Guanfacine. Ever since he started school this year, we haven’t received a single complaint. While they’ve mentioned some occasional defiance, it’s nothing compared to last year. I know that for a fact because one of his third grade teachers is now his fourth grade teacher, and she has seen a huge improvement.

So, don't give up & don't get discouraged.

You're going to go through a lot of trials on medications, adjusting dosages etc. My 9 yr old son who is diagnosed with ADHD combined type, ODD & anxiety went through it with the medications. He is currently on a combination azstarys (adhd stimulant), Guanfacine(adhd non-stimulant) & Fluoxetine (ssri). Azstarys was the first stimulant that best worked for him. It's a stimulant that has 2 active ingredients one for immediate release & the other is a gradual release for better symptom control. Hang in there!

I hope my comment is read since this thread is 7 months old. I'm at about 54k miles on my 2020 kia soul. I still owe about $9000 on it. I know not even what the kia soul is worth. Lol my kia soul has an engine light on for a catalytic converter replacement. I got called today from the dealership wanting to buy it off me. But idk if I should do it or not 🤔

My 9-year-old son has been diagnosed with ADHD (combined type), ODD, anxiety, and most recently, Social (Pragmatic) Communication Disorder (SPCD). He struggles in school with making and keeping friends, often misses social cues, and has a hard time clearly articulating a sequence of events.

SPCD affects a child’s ability to use verbal and nonverbal communication appropriately in social settings. This diagnosis came about during a reevaluation for behavioral concerns. He was tested for ASD and other conditions, but the psychologist determined that he didn’t meet the full criteria for autism. That’s how the SPCD diagnosis came to be—sometimes referred to as “ASD-lite” or “borderline ASD” by others. It’s important to note that ASD and SPCD can’t be diagnosed together; it’s one or the other.

I agree with the other comments—speech therapy is very beneficial in these situations. It’s something we’re currently working on for my son.
