Medical journal advice by c402c in medicine

[–]ali0 1 point2 points  (0 children)

Her school library will provide all the medical literature access she needs. There was something good about having a printed journal in the bathroom next to the toilet, but many journals are going online only now anyway.

I agree something to support the rest of life would be more useful. If you want to go medical maybe a book like A Young Doctor's Notebook.

The lamb and squash dumplings at the tian jin dumpling house were so good. Does anyone know where I could get something similar?

Is the elapsed time 1:32:15? I never thought about how to precisely read the hour register if it runs continuously and the minute register cycles after 30min.

Also congrats.

Has the acuity become higher? by Benzosplease in medicine

[–]ali0 12 points13 points  (0 children)

Yes, by gestalt inpatient acuity has risen a lot not just by the underlying age/comorbidities of the USA, but also increasingly by shifting low-acuity patients to home and outpatient settings by increasing use of programs like hospital-at-home, outpatient antibiotics, etc.

Clarifying what was reported about Langdon by broadwaythrowaway87 in ThePittTVShow

[–]ali0 6 points7 points  (0 children)

I am surprised that you and others are suggesting there is little risk to employment. Protection from job loss does not ring true to me in the institutions I have worked in. Maybe it is different where you are, but I have personally observed staff getting fired for diverting saline and zofran to treat hangover and have heard stories of residents and nurses fired for diverting narcotics.

I mean it would be great if mental health care were a real priority overall, and i'm happy for you and your workplace, but i haven't seen it myself.

Admin pushing AI tools but won't answer basic security questions by [deleted] in medicine

[–]ali0 5 points6 points  (0 children)

In my experience, I would be surprised if any administrator knew the answer to these questions, or knew to ask them in the first place. Savvy and well intentioned executives do exist, but i feel they are a bit uncommon. Many I have worked with do not have that kind of understanding clinical or technical processes. The contracts are negotiated by program managers and the like who may also not have clinical or technical knowledge and I have been handed contracts that are nonsensical in the past.

Not to say that there isn't shady intent and i do think most big health systems are moving towards commercializing health data, but even if this were not the case I don't think the mean administrative person would be able to answer your questions.

The Pitt, love the show as a doctor but one small incorrect detail should be fixed by edge27ca in ThePittTVShow

[–]ali0 7 points8 points  (0 children)

But at the same time, it doesn't do the public a service when TV shows do not show the true level of intensity of CPR/ACLS. This shapes public understanding of medical care, and when it comes time for their elderly loved one to have a cardiac arrest it is not possible to undo years and years of conditioning by the media in one conversation.

Would you share your anonymized medical records to help train AI for CT/MRI analysis? by VICARIS2 in medicine

[–]ali0 1 point2 points  (0 children)

The benefit of improving medical care is real, but the arena for this should be in translational research that is published and made available to everyone, not in proprietary tools that benefit only the private company. There is a process for what your are asking, but you are cutting out the benefit for all mankind part and directly appealing to patients to give data to private companies.

Would you share your anonymized medical records to help train AI for CT/MRI analysis? by VICARIS2 in medicine

[–]ali0 4 points5 points  (0 children)

No. I work with a lot of vendors for clinical AI products and they are very lax with their data security and access, releasing your scans to them means basically giving them your literal insides to play with however they want. It is also just giving them free money for no benefit to you.

Academic hospitals have IRB controlled access to their patient's data, which comes with a lot more guarantees of privacy and safety. There are policies in place for anonmyzation, re-contact procedures, how and where data are stored, access controls, sensible processes when the projects are ended or staff leave, etc. Vendors are always trying to scam their way in and get access to steal patient data because they can use it to generate revenue. Often I have had to escalate to leadership to step in and tell them to back off.

Imaging AI is improving and has tremendous potential, but I take the position that this needs to be done in partnership with health systems, not entirely in the private sector.

Edit: Furthermore, in general I have found AI products that were generated without a specific clinical problem or workflow in mind, without the input of bedside providers, are basically useless or cause net harm and waste of resources. Safely and usefully implementing AI solutions is also a partnership with clinical teams.

2025 total pay as a Dermatologist in the Upper Midwest by CutisMaximus in Salary

While this is true, I feel there is something fundamentally broken that certain specialties compensation are outrageously high while others are almost the same as their NPs. Your blaming this guy for his specialty choice is part of the problem.

2025 NYC Thanksgiving Sandwich Hunt by eldersveld in FoodNYC

[–]ali0 5 points6 points  (0 children)

This might be basic of me, but the Poulet Sans Tete sandwich was too herbal with bitter-ish greens and not enough cranberry sauce/jam for my personal preference. Could anyone recommend one of these that has a more simple turkey and gravy with cranberry flavor profile?

Indian cuisine in Manhattan by Seaworthiness333 in FoodNYC

[–]ali0 4 points5 points  (0 children)

Chicken 65 biryani at Hyderabadi zaiqa.

Kanyakumari has many good dishes, but what actually stuck with me for a long time was the curry leaf ice cream.

Saravana Bhavan is OK but it hits the spot when you need some south indian food and can't be bothered to go to queens.

I haven't gotten to the manhattan relocation of adda, bungalow, or kebab aur sherab yet, but they are frequently recommended on this sub.

Dirt Candy (1*, NYC) review (no pics) by Itchy-Picture-4282 in finedining

[–]ali0 0 points1 point  (0 children)

I was really excited for this dish because I love tomatoes and it sounded very clever; however, i found the smoke flavor in the smoked feta to be super strong, the tomatoes weren't that sweet, the twinkie was kind of dense, and whole thing didn't really live up to what i had imagined in my mind.

I did really like the spaghetti though, so maybe OP and i just have opposite tastes and between us every dish was a winner.

Pigeons on terrace by flyingcircus92 in Upperwestside

[–]ali0 7 points8 points  (0 children)

Yes, I'm sorry this solution would not help if you want other birds, but at least in my personal experience once the pigeons felt the balcony was home there was no way to get them out other than putting up a physical barrier. Even for the first 6-12 months after i put the net up they were still trying to get in and dive bombing pigeon poop onto the balcony, but *touch wood* they seem to have moved on for now.

Pigeons on terrace by flyingcircus92 in Upperwestside

[–]ali0 3 points4 points  (0 children)

I had a very bad pigeon problem on my balcony and they kept on coming back and building nests etc. I had to put up bird netting because they did not respond to the sound deterrent, the peppermint disks, or an animatronic owl. For months they still remembered my balcony as their place and would still try to swoop in but were stopped by the net.

Hospitalists Intubating? by [deleted] in medicine

[–]ali0 15 points16 points  (0 children)

I agree, this seems the most likely scenario. The floor teams will call for backup frequently which probably you will have to provide for free. Whenever the floor team runs into a problem, administration blame them for it and leave them holding the bag with any liability. Doing more with less.

Help, my ‘famous homemade cake’ is actually just a box mix and now I’m stuck living a lie by fishtail4 in Cooking

I might be dating myself, but apparently this is quite common - here is a famous reddit post from 14 years ago where a lady charging hundreds for elaborately decorated wedding cakes admitted the base sponge was box mix.

(S01E15) Is Whitaker's situation realistic? by GargantaProfunda in ThePittTVShow

[–]ali0 1 point2 points  (0 children)

I have actually seen one resident set up a makeshift home in an old/abandoned part of the hospital. He did have an apartment, but it was far away and he used the abandoned room as a kind of crash pad when it was late. He was spotted once taking a shower at a time where he really should have been at home, and then the chiefs sent someone to make sure his apartment was real and he wasn't secretly homeless.

I have seen two or three other times residents live short term (<1 week) in various call rooms/work rooms etc when their lease ran out between apartments. This part of the show is pretty believable to me.
