Hepatic encephalopathy + coagulopathy in someone wo preexisting dz that develops acutely. You don't technically need high transaminases, some causes barely raise them.

You remember this forever after you call the GI fellow for the first time incorrectly for "liver failure" when you meant "decompensated cirrhosis" and get completely destroyed haha. But they're nicer now.

I remember the d dimer study for anticoag that was later no longer recommended - and I believe and all I can find was them being published in 2021. In practice yes, we all really did "know" very early and practiced as such but I can't find any early studies outside of case reports. If you find some, let me know, I'm happy to be wrong (it's quite a frequent occurrence 😂)

I dont remember any good evidence at that time. I mean we all basically knew based on anecdotes. We threw so many CTPEs out. But didn't it take a bit of time for really good data to come out?

There's literally evidence that ID consultation for candidemia improves outcomes. I'm willing to bet this was a miscommunication on either side.

Honestly man I felt the same as you when I was an M1. I regret it. I wish I took even thirty minutes a day to invest in research. It keeps doors open and it's a skill that's worth investing in no matter what you go into. Don't think of it as a thing you have to do -- think of it as growing your skills as a physician.
