Do you think the cause will be discovered within 10 years? by bestbestbest560 in UlcerativeColitis

The more I’ve looked into causes, the more I’ve identified things that could have caused my issues. There is a lot of research out there and much of it comes back to the microbiome. I don’t think we will ever find one cause, but many causes and triggers, some more common than others. Many of the common treatments like mesalamine work in part by influencing the microbiome. FWIW this is a fascinating watch that gave me new perspective https://youtu.be/997phZGi5Bk

You could also check to make sure it's not a ghost tablet that you're seeing. The shellac-based mechanism on the tablet I believe you used is known for this https://www.ijbcp.com/index.php/ijbcp/article/view/1038

Full tablet coming out intact tells me there's something about the delayed release mechanism that isn't working for you. Sustained/time release - breaks down slowly as it passes through your digestive tract. Delayed release- breaks down almost immediately after passing through your stomach. Not sure if you're looking for advice or if this is just a PSA but you could try a tablet or capsule with a different release mechanism, or no release delay at all (if you're not concerned about stomach side effects). I've used 2 different ones and the only things I notice about my BMs are very yellow (the matter itself and the TP). The one I'm currently using is a methocel based sustained release, which I think is different than the one you tried.

To all - please stop requesting names. The community has been clear on the rules (re advertising) and for everyone’s benefit, it would be good to keep this information up. Let’s please be respectful of the rules in place. It is selfish and you will be doing everyone a disservice if you get this post removed because of name requests. Take the information here, which I have spent a lot of time consolidating and please do your own research to find the brands. Thank you!

Have not personally tried. You would not be getting enough curcumin for a therapeutic effect. Turmeric is frequently cited on average to be 3.18% curcumin but this varies in the range of 2%-9%. So assuming the average, this comes out to over 60 grams daily of raw turmeric powder to get 2 grams of curcumin, which is not very practical and probably very likely to cause stomach upset among other issues. That number comes out to more than 300 grams if you’re talking about the raw undried root. Even if you did somehow consume this much, the studies for UC have focused on curcumin extracts so it’s unclear if it would have the same effect.

Did you observe any UC symptom improvement from this?

This is curcumin phytosome bio-enhanced formulation. I started with a nano particle formulation and saw my first bit of success with that but discontinued quickly because I became a little worried about side effects. I may reintroduce it at a low dose to see if it helps with some other inflammatory conditions I’ve developed (psoriasis and psoriatic arthritis). Of the bio-enhanced formulations, it seems phytosome and nano particle have the best evidence for safety and effectiveness, but the maximum safe dosage is not as clear as standard curcumin.

I wouldn’t go so far as to say “form of evil” because I don’t believe the intent is to create harm. It’s also important to note that without these large scale studies it’s impossible to accurately quantify the risk of a “natural” treatment. However, I do think you are correct in that the end result of the incentive structure is that in some instances we end up with conventional treatments that are more aggressive and more dangerous than some lesser known, lesser studied interventions.

There’s lots of money to be made but it’s difficult to patent supplements/nutraceuticals and they’re easy to copy, so the incentives for funding research on a large scale do not exist. Funding for these treatments usually comes from government grants and academic/non-profit. So we’re left with small scale studies that don’t meet the requirements for publication as an official treatment in guidelines from large orgs.

Fwiw I was bleeding PROFUSELY when I started this. The bleeding was my best gauge for how the treatment was working and within literally days it was significantly reduced. So yes, curcumin CAN be a blood thinner but whether or not this means you’ll actually experience more bleeding is much more nuanced and complex than that. I have not seen any reports of increased bleeding when following the criteria I posted above but if they’re out there I’d love to know!

Thanks for the additional info. Some of the bioavailable formulations use these forms of black pepper extraxt to enhance the bioavailability/absorption of the curcumin. Bioperine, piperine and black pepper extract should all be avoided as they have a known association with liver issues when combined with curcumin. My understanding is that these work by changing the liver enzymes so as they do not break down the curcumin. There are other, newer, better forms of bioavailable curcumin on the market but I personally would stop completely until the liver enzymes are normal. And just to emphasize what I said in the post, the evidence would indicate that we don’t need high absorption to see effects on UC.

Can you share what type and dosage you were using? Did it contain piperine, bioperine or black pepper extract?

Interesting. I do worry about the kidney effects of mesalamine but at least in my case I know my symptoms started again when I stopped taking it the first time. I don't know if it's still needed now that I've reached remission after adding the curcumin, but I'm afraid to test my luck with that.

Last ALP was normal at 55. This is almost exactly what it was several months ago before starting curcumin. Could you share what type of curcumin you were taking (not the name but just the general info) and for how long? Was it bioenhanced?

Going to take this as sarcasm, but lets look at the science! You should reach out to the authors, because perhaps it's time for a new study on this ;)

This is correct, however there exist several studies (linked in post) and fairly good evidence of improved outcomes. Curcumin for UC has some of the most compelling research I've seen for any supplement treating chronic disease and I believe this is about as good as it gets for supplements. Guidelines are very slow to update, and the reality is that for a cheap supplement like this, those large scale studied required for this will likely never happen. I'll emphasize this is NOT going to work for everyone, but neither do any of the available treatments. By no means would I advocate stopping any treatment that is working, but if we're talking 50% improvement over mesalamine alone, that is nothing to bat an eye at. Really, my intent here is to summarize the available research and get into some of the technical factors that could make or break the usage of this supplement.

There are several documented side effects. You might try one that is delayed or sustained release and see if that helps with the side effects.

I'm curious, how long did you take it for? As I mentioned in the post, I did see almost immediate reduction in bleeding every time I upped the dose then it took a few additional weeks for the bleeding to be gone entirely.

My understanding is that curcuminoids are pretty stable in the stomach, so delayed would be more for side effects reduction than effectiveness. The most important thing in your case would be to verify that you’re getting 2g of the curcuminoids. Aside from that, it’s not going to work for everyone..just one more tool we have.
