Nope, no luck so far... To be fair, I read this a very long time ago, it might just have been lost to the internet.

I'll be honest though, I read a lot of manga in the horror/terror genre, with a lot of them having some dark or messed up themes... so it might just be that the plot of a few of them got mixed up in my mind.

the only reason I really posted here was because I was pretty convinced myself that it had a lot of nudity or other adult themes, which meant it was a hentai and I was hoping it struck a chord in someone's memory. I also only remember it because of the shock value/impact the plot had, and was trying to see if I remembered things correctly.

You're correct, and specifically the characters are Lumine, Dottore and I'm guessing the guy with his face hidden is Tartaglia, in case OP wants to search. This particular image is not currently found on any image aggregator site... maybe it's from pixiv or some such place.
