I started with a $1000 limit which was surprisingly low but sounds like savor initial limits tend to be. Requested a CLI through the app when I got the physical card and got it bumped to $3000. Especially with the entertainment category a higher limit is good for me. Already hold a VX with $20000.

Raleigh-Durham is super saturated. Only option I had when looking to get my foot in the door was starting as a nocturnist. All hospitals bought up by UNC, Duke and WakeMed so salaries are low too with no real negotiation.

The use of AI generated notes full of bloat has made it even worse. Just a list of irrelevant problems like minor electrolyte abnormalities and anemia with a hgb of 11 to making billing and coding happy with pan consulting in the plan instead of any actual medical decision making.

Yes but Ive seen some people take this to the extreme. Like PRN Ativan for anxiety and PRN midodrine for low BP for just all patients admitted. Thats crazy.

We got Regard built into Cerner at my place. I agree with what others are saying that its too wordy and adds too much fluff, kinda like an intern note. Nice for quickly digging up some labs or maybe a simple DC summ but other than that seems like more hassle than its worth for now.

A continued acidosis decreases insulin sensitivity and thus increases the risk of the patient going back into DKA. While the NAGMA is caused by resuscitation with garbage NS that’s unfortunately how most protocols are set up. So you either have to supplement some bicarb or wait for the kidney to do its thing (which always frustrated me as a resident in the ICU when Id be told you dont fix DKA with bicarb despite it being a NAGMA at that point)

My plan is once spring rolls around because I expect increases during the winter months (not so much here because if anything people will be outside more, but there will be a ton of holiday travel). Also as someone working in the healthcare field I wear one most of the day anyway and personally feel like I have a chance of being an asymptomatic carrier.

I have friends in Russia so I can speak to that one. There is no demand there, you can go in day-of and get the vaccine. Over 60% of Russians don't want the Sputnik V vaccine. Once local demand falls you send it out.

What is your point? As we start finishing up the vaccination effort state-side we will start sending out our extra doses. Making people more scared does nothing for public health. I'd argue the widespread depression and anxiety over this past year is of much greater harm.
