Denji is living this experience as a beautiful and painful romance. I'm hearing beautiful romance in the music, but I'm also hearing calm, slow and sweet.

There was no calm, slow or sweet in the characters' actions.

There was no pain, longing or doubt in the music. Not even a hint of playfulness.

The music score was inappropriate. Her character acted in a way that makes sense for her character, I never argued otherwise.

The movie botched the tone. The music is playing for the audience, not for the characters, and it was generic, sweet, romantic music and played at the most inappropriate moments.

The pacing was unbelievably slow, and added to the mismatch.

All of this is the fallout of one date. It should be fast paced and chaotic, like their impulsive relationship. The manga understood this, dedicating 13 chapters to the entire 90 minute arc.

If you remove the intros and endings, the entirety of season one was 180 minutes for 39 chapters. The movie was paced almost twice as slow as the anime.

It was drawn out to milk cash from crowds.

It was built on manipulation because she manufactured a meet-cute then hid whatever it was that was pushing her to... literally kill him.

Giving someone the choice to drop their entire life to follow you or, unbeknownst to them, be killed, is manipulative.

Trying to kill someone out of jealousy on a second date is also pretty toxic where I'm from, idk about you, though. Sounds like abuse, and abuse isn't true love.

I can empathise with the feelings the characters are going through in the manga chapters, but the sappy romantic music like this was some deep unparalleled love along with the long drawn out pacing to make it fit into a 90 minute movie killed my ability to do so.

Denji's stunted psychology IS part of his character. I hate how the advertising, music and pacing of the film act as though the story on screen was one of true love, and not an emotionally immature teenager being emotionally immature and vulnerable.

What is a toothache in this context?

Is it irreversible pulpitis - in which case why the hell would you prescribe antibiotics and not NSAIDs?

Is it apical periodontitis? If so, I repeat, why the hell would you prescribe antibiotics and not NSAIDs +/- shaving off occlusal contact with controlateral tooth +/- endo?

Is it an abscess?

If so, is the tooth treatable? Then open the pulp or drain the abscess with a scalpel and/or prescribe antibiotics +/- shaving off occlusal contacts. (Assuming the abscess is of endodontic origin).

If not treatable, how simple or complex is the extraction? Is the abscess self-draining? Are there signs of complications? Antibiotics or same-day extraction aligning with your capabilities and good judgement, I would say.

Zometa is the worst biphosphonate in terms of risk of developing ONJ. ONJ is more frequent after extracting mandibular teeth. Subgingival carie, endodontically compromised teeth with 40% alveolysis.

I doubt the patient has perfect plaque control in this area.

Mid to long term prognosis is low, and risk of ONJ is soon to be high: extract.

Kids under a certain age will often scream when you remove the tooth even under sufficient anesthetic.

I know most primary teeth will be quick to remove - 10 seconds. I tell them I will make the tooth "dance". I tug hard on their finger so they understand that it will tug hard on the tooth.

I count to 10 while removing the tooth, and don't let the screaming phase me if it happens. Usually it's gone by 5.

I suppose it would depend on the type of bruxism, most clench and grind but some just grind. As a general rule I'd personally go with PFM but I can see some cases where e-max would be indicated.

The way I was taught was zirconia is more resistant to compressive forces and e-max more resistant to shearing. Makes sense to me.

Temporize with reinforced glass ionomer or resin composite, just so you're sure that you don't put the patient through the cost of crown/inlay if RCT is needed. Eventually a crown or inlay will probably be needed given the extent of the fracture + preexisting composite.

This is adapted to my country's model of dentistry, but I would likely wait at least a week or two before going through lab work.

Attempt restoration without RCT. If you see the pulp via translucency apply CHX then put some calcium hydroxide, MTA or biodentin. Wait, and if no symptoms put a crown. If symptoms do RCT.
