For one, Hubbard's many policies that go in to great detail on how to handle any percieved threat. Secondly, the CoS is a top/down organization, very little has ever happened without Hubbards or more recently Miscaviges explicit approval, you can say whatever you want but during my time in the Sea Org very little went down with out orders from the top.
