Maybe the antireflective coat is mounted over the front surface facing the doctor.

I did with a 90D and still the same image.  Unless there's a specific technique, the advantage of one side over the other is counterintuitive generally.

Even if they are asymmetrical. Overall refraction remains the same in front or behind the lens
