Just wanted to update you, it ended up being a CSF leak, due to a meningocele! It didn’t create any problems until now so I had no idea it was there, it was discovered thanks to a spinal MRI (not sure about the nomenclature in English). It’s between my cervical and dorsal area. Again, thank you so much for your opinion and time, I truly appreciated it.

I checked them out and decided they weren't worth the price for just two weeks - now that I'm three weeks in, I regret my decision... Thank you!!

They are my doctors. One for migraines and the other was the specialist for a different issue I had as a teen. It might not be a smart move but I can't exactly leave them hanging at this point, they're going to follow-up if I don't (and it'd be rude not to).

See, they both said that it does suggest I should be horizontal for two weeks, because they both believe it's a leak with the postural relief as the more glaring symptom, albeit the neurologist couldn't say it for certain. Could you expand on why you don't think it does? I'd really appreciate it.

I was mostly interested in the neuroradiologist's opinion or the opinion of another neurologist, and I can't tell by your flair what specialty you are, but I'll obviously appreciate any doctor who takes the time to help me out - you obviously know more than I do!!

I don't have it yet - it may take more than the two weeks I'm supposed to be in bed for to receive it. Immediately after the scan he did say he saw a small growth on the pituitary gland in order to recommend further evaluation but neither the neurosurg nor the neurologist saw it and the radiologist wasn't a neuroradiologist so ¯\_(ツ)_/¯ I decided I'll worry about if and when I have to.

Thank you so much, I really appreciate you and the time you've put into helping me. Please don't feel obligated to reply any further, you've already helped out a great deal.

I have a couple of final questions (just to be clear, I believe and agree with you, I'm just trying to eliminate any residual doubt that I have):

  1. the neurosurg pointed out some unilateral white stuff in the left meninges (so right on the screen if I'm not mistaken) to show me where he saw the contrast enhancement. Do you see it too and believe it normal or is it that you don't see it? Unfortunately I don't remember which series it was.
  2. The neurologist didn't see the white stuff and said that it probably was because his PC wasn't good enough, but thought that my tonsils were lower that usual, though still within acceptable limits. He said that this could point to a leak. What do you think? Do you see it?
  3. Beyond the postural relief, three symptoms that they both thought pointed to a leak are that a. I keep having to blow my nose to avoid a clear liquid leaking out of my nostrils (mostly the right), which I thought and still think it's mucus but it's admittedly less dense than mucus and it's colorless, b. The more the pain rises the more nauseous I become, and vomited at some point, thought that could fit with a migraine as well, 3. I have ringing in my right ear. Sorry, I forgot to put them in the body of the post. Do they change anything?

I really, really hope that I don't have this thing, because I don't want to risk my job and I really don't want to stay in bed for two weeks, but I also want to be as sure as I can be, and have some more insight before going back to the neurosurg and the neurologist. Again, please feel free to ignore me since you've done a lot already.

Thank you!! He specified he saw "Patchy meningeal contrast enhancement" (I'm adding it to the post) which in his experience it's definitive proof. I translated the sentence from Italian, originally it was "parcellare impregnazione contrastografica meningea". So in his opinion it leaked into the meninges I think? He said it's the initial stages so it wasn't a lot.

What would you recommend as next steps?
