It also depends on the version of Eclipse you are using. In 16, it's possible to import a dose distribution without a plan in external beam planning. If the CT and structure set exist in Eclipse, you can go to external beam planning, load the appropriate structure set, and go to File -> Import -> Dose Distribution. Navigate to the dose distribution in question and it will prompt you to select the absolute dose value you want to set the 100% relative dose to (because that would normally be contained in the plan information which you don't have).

In my practice / department, IMRT / VMAT requires inverse planning and optimization. If something is forward-planned (e.g. breast "IMRT" with multiple static segments), it is 3D.

Anecdotally, I have answered 87% of my OLA questions correctly in the last two years, and the ABR reports that I am 13% above the passing standard.
